You have a right to express a preference of a particular doctor to register with. This request will be accommodated where possible but will depend on the availability at the time. Please indicate this to staff at the time of registration.

Dispensing or Non-Dispensing
As a dispensing Practice, we are not allowed to dispense to patients who live within 1.6km (1 mile) of a pharmacy. If you live within one of the dispensary boundaries shown on this map we can’t dispense to you.
Proof of Identity and Residency
Proof of identity and residency is required for all adults who wish to register at the Deepings Practice; this does not apply to those aged 17 years and under.
In order to confirm your identity and proof of residency you are required to bring one of the following documents to the Practice Surgery:
- To confirm your identity, please bring either your Passport, Driving Licence or Birth Certificate; and
- For proof of residency, please bring either a recent Utility Bill, Council Tax Bill, Bank Statement etc. which shows your name and current address (proof will only be accepted if the name of the person wishing to Register is clearly shown on the bill or statement).
If you are unable to provide evidence of identity please contact the surgery for advice.
If on repeat medication you will be asked to make an appointment.
H M Forces - Service Leavers
To facilitate the transfer of Service Medical Records, patients who wish to register having left the Armed Forces must provide a FMED 133 – Medical History on Release from HM Forces Form which is available from your Service Medical Centre.
You are also required to complete:

Become a member of our Patient Reference Group
The Deepings Practice has a well-established and valued Patient Participation Group who meet regularly within the Practice. The group discuss items of mutual interest to the practice and patients, have formed a Voluntary Drivers Scheme and maintain a positive, effective link between the Practice and its patients, ensuring the Practice remains accountable and responsive to patients’ needs.

Zero Tolerance towards Physical Violence and Verbal Abuse
We operate a Zero Tolerance Policy towards physical violence and verbal abuse in order to protect our doctors and staff. lf a patient becomes violent or abusive towards any doctor, Staff or other person present on Practice premises, we reserve the right to arrange their immediate removal from our list.
