When you are discharged from hospital, if required, you should normally receive a supply of medication.

The hospital should issue you with a list of your medicine requirements (discharge documents) on departure.  Please arrange for this to be handed in to the surgery well before your supply of medication has run out.

Hospital requests for change of medication will be checked by the GP first, and if necessary your doctor will issue you with a prescription. The Practice will aim to issue you with your prescription within 3 working days.

The Doctors will review your medication regularly, which may involve changes to your tablets, in accordance with current policies. 

Outpatient Prescriptions

If you are issued with a white hospital prescription following an outpatient appointment, it can only be dispensed at the hospital pharmacy.

It may be that the hospital will have to order in items they do not stock, but this is something they should be offering you.

You will not be able to get it filled at either a local pharmacy or our in house dispensary.

If you are supplied with a green hospital prescription these can be dispensed in the community.
