Clinical Phamacists
The Clinical Pharmacists support patients in how to take medicines in a way that enables them to get the most benefit from them. This includes providing information, ensuring that your medicine suits your medical condition and symptoms, using their evidence based clinical knowledge.
They provide a listening ear to discuss all medicines related queries and concerns that you have, whether it is to do with the best time to take your medicine, through to a discussion about side effects or whether your medicine is right for you under changing life circumstances.
As part of your medication review, they will ensure that your medicine is working safely for you through monitoring by requesting blood tests where needed. If there are medicines that you have used for a long time, they will discuss these with you and work with you and your doctor to establish if they are still needed.
They will undertake reports for different medications to ensure the correct information is being provided or monitoring is being carried out for specific medicines or groups of people
If you are seen in hospital, it is part of their role to ensure that new or changed medicines are added to your medication record after seeing the discharge or clinic letters.
