Booking an Appointment in our Travel Clinic

Please note that the practice has no obligation to provide last minute travel clinic appointments and due to demand, we usually cannot help at short notice.

We strongly recommend that you book appointments at least six weeks before you travel as vaccination does not give immediate effective coverage.

To book a travel vaccination appointment you must complete and submit the pre-travel questionnaire; the way you submit your Pre-Travel Questionnaire form has now changed.

Please use the Contact Us Online form to request a pre-travel Questionnaire; selected the “I have an admin query” option followed by “I need help with something else” and enter the text “pre-travel Questionnaire” in the box.

Upon receipt of your request, you will be sent an SMS or email requesting you complete a questionnaire; this questionnaire will ask you some questions about your upcoming travel plans. This information will help the healthcare team review your health needs related to your travel. It should take no longer than 5 minutes to complete.

A questionnaire needs to be submitted for each traveller in your family.

Once you have submitted the questionnaire a member of the practice will be in touch with a telephone travel appointment for a risk assessment to be carried out and if any vaccinations are required a follow-up appointment will be made. Please be aware that some travel vaccinations are chargeable and if these are required payment will need to be taken prior to administration. This will all be discussed with you at the telephone appointment.

Please ensure that the Practice has your up-to-date contact details.
